"May the divine power of nature bring forth strands of life to form a body for this wayward soul!"

You use the power of nature to form a new body for the soul of a dead humanoid to inhabit.

You touch a dead humanoid or a piece of a dead humanoid. Provided that the creature has been dead no longer than 10 days, the spell forms a new adult body for it and then calls the soul to enter that body. If the target's soul isn't free or willing to do so, the spell fails.

The magick fashions a new body for the creature to inhabit, which likely causes the creature's race to change. The DM rolls a d100 and consults the following table to determine what form the creature takes when restored to life, or the DM chooses a form.

The reincarnated creature recalls its former life and experiences. It retains the capabilities it had in its original form, except it exchanges its original race for the new one and changes its racial traits accordingly.

Coming back from the dead is an ordeal, and the creature needs to get used to its new body. The target takes a –4 penalty to all attack rolls, saving throws, and ability checks. Every time the target finishes a long rest, the penalty is reduced by 1 until it disappears.

Variant Resurrection Rules. In order for this spell to work, a resurrection ritual must be performed as part of the casting time of the spell. Up to 3 allies can make a contribution to the ritual, which usually signifies some kind of relationship they have had with the person, as they beseech the soul back to the land of the living. The DM determines what kind of ability check needs to be made, along with the DC, based on the contribution.

Then, the DM makes a resurrection check, rolling a d20. The DC starts at 10 + the number of times the target has been brought back to life. Then, the DC is increased by 1 for each ritual contribution that failed, and it is reduced by 2 for each contribution that succeeded. On a successful check, the resurrection ability is successful, but on a failed check, the resurrection ability is unsuccessful, and the creature can no longer be brought back to life.
